Appointments
Patients are seen by appointments only. Patients who miss their appointments should contact the front desk. She will review circumstances and reschedule the appointment on the same day or different day. Walk-ins are not permitted.

Intake Policy
A driver's license, ID, Physical address & a phone number are mandatory. The Tennessee Professional Associates has the right to provide or deny service to any individual entering the clinic. Each patient is considered on an individual basis. Individuals with improper, filthy attire, or under the influence of drugs and/or alcohol, etc. are not permitted. Food, alcohol, using cell phones, smoking, loitering, etc. in the premises and the parking lots are not permitted.

Consultation, treatment, prescription, and referral policy
The Tennessee Professional Associates strictly adheres to local state and federal guidelines. The Tennessee Professional Associates does not believe in "on line" or "telephone" consultation to treat patients. Face to face consultations, physical examinations, relevant laboratory and radiological investigations are necessary to prescribe medication.

Termination Policy
The Tennessee Professional Associates has the right to terminate any patient without any reason or based on several factors, which include forging, fraud, criminal activities, altering prescriptions, substance abuse, failed drug screens, failure to comply with the instructions, etc. Termination will be with or without notice depending on the circumstances.

Payment for Professional Services
Due to the low cost of our service, payments are due at the time of service by cash, cashier's check, money order, or major credit cards. A picture I.D. is mandatory to use the credit card.

Release of Records
Copies of the patient's records require a completed release authorization form and a fee for copying and handling. Records will be release with in ten (10) working days.

Privacy Policy
Our Privacy Practices of Protected Health Information (effective April 14, 2003) are in accordance to the Health Insurance Portability and Accountability Act of 1966 ("HIPPA")
